Adoption Application Form

Before you fill in our adoption application form, ideally you will have a dog in mind that you are applying to adopt. Although you can always get in touch with us to find out if there is a dog who’s the perfect match for you, we are very good at matching families and dogs.

  • We only rehome dogs from Cyprus to the UK and we have some dogs already in the UK
  • Before completing this application form, please ensure you have read and understood the Terms and Conditions and Privacy Policy and understand our adoption fees.
  • Adopting a dog is a long-term commitment (10-18 years in the case of a puppy); please ensure you can take of the care of the dog, both practically and financially, before deciding to submit your application.
  • Complete all parts of this form and submit it. Alternatively, if you wish to post this form, please request an address from the member of our team that sent it to you. We can also email you a form to complete and send back to us.
  • We need a photo of ID (passport or driving license) that the home checker will use on arrival to confirm they have the right home/person. This can be sent after you have submitted your application form when one of our volunteers contacts you.

Adoption Fees and Transport Costs

Adoption Costs:

We ask for a fee of up to £375* for dogs already in the UK and a fee of up to £650* for adult neutered dogs traveling from Cyprus to the UK. This contributes towards the costs accrued during preparation and travel to the UK including neutering and spaying for dogs old enough. Prior to leaving Cyprus your dog will have had the following tests: Snap 4Dx which tests for exposure to pathogens that cause Heartworm disease; tests for Ehrlichiosis, Lyme disease and Anaplasmosis; and Leishmania snap test. The dog will also have dog/puppy vaccines, rabies vaccine, microchip, worming treatment, tick and flea treatment, a fit-to-fly examination and an EU-pet passport.

For dogs aged under 12 months the same preparation takes place (minus neutering/spaying) so their adoption fee is £600. There will be a requirement to have the dog spayed/neutered by your own vet, when it reaches the age recommended by your vet for doing so, at your expense.

*Microchip registration is £20 on top of the adoption fee, which is carried out by the rescue team on your behalf once the dog is in your care.

Payments:

A payment of £100 will be requested to reserve your dog. If your home check is not approved, then the full amount will be refunded. The remaining payment will be requested after notification of a successful home check. Payment details will be provided on request:

What Happens Next?

What happens next?

  1. All Adoption Application Forms are reviewed with the sole intent of making the best long-term companion match possible. You will be informed as soon as possible as to whether we feel you are a match for the dog.
  2. If you would like a form sent to you to complete we can post one or email to you.
  3. If you are successful and go onto adopt one of our wonderful rescue dogs please join our adopters Facebook group Saved Souls so we can keep updated on their new life.
